Considerations

• Are there forms already in the system that you would like to add to your collection?
• Are there additional forms that you would like digitized?  Is there a different version of an existing form that you need to use?
• Do you have a higher quality PDF of a form that already exists?  (We'd be happy to update our library)
• Are there any internal forms that you would like to include in the system?

How Form Conversion Works

Any form that's already in the system can be used by your organization by simply selecting the form from the Form Library and clicking "Add to Collection."  Once this is done the form will be available to be included in any activity.

If there are forms that you would like to have digitized, please provide a high quality PDF to the HealtheFirst support team, and they will get to work on the conversion.  Please also indicate if the form is proprietary to your organization, so our team understands if the form should be kept private in the system or if it should be visible in the public library.
